What to do in case of claims?

NB! According to clause 6 of the Cavitas dental insurance terms and conditions, certain treatments (crowns on multiple teeth, bridge(s), denture(s), implant(s) including bone augmentation and sinus lift procedures) are indemnified only if the claims administrator has given the confirmation before starting the treatment.

For a approval of above mentioned treatments please ask your dentist to fill in the pre-approval form what you can download here.


Receive dental treatment

Recieve dental treatment at your preferred dental clinic.
Request a detailed invoice from the dentist which includes the following:
• name and address of dentist /dental practice and practice number
• name of the patient (you) receiving treatment
• treatment date
• description of each treatment received and the treatment code
• fee charged for each treatment
• any deductions for Sygeforsikringen "danmark" (Health Insurance "danmark" association) or the public health insurance
Pay for the treatment performed and keep the payment receipt.

Enter claims portal via this website menu option "Claims" (https://cavitas.dk/claims)
When you first log in, do register yourself as a portal user. To enter every next time please use only your personal code and password you created during the registration process.

Upload documents to claims portal

Upload the dental invoice and proof of payment to claims portal by following the on-screen instructions.

Wait for the decision

Wait for the claims administrator's decision. The processing of your claim may take up to 2 weeks.
Keep track of your claim status in the claims portal yourself.
An indemnity notice will be sent to your email address to inform you of the decision of the claims handling process.

Payment of indemnity

The insurance indemnity will be credited to your bank account within 5 working days of receipt of the notice of claim payment.
